Are you ready to join us?

Great! It is simple and easy. Come on down to the paa, have a chat with our doctor onsite and bring your identification to enrol with our clinic. Listen here to as Hads guides us through the process.

What to bring if you are enrolling

You will need to be enrolled with our health partner Te Awamutu Medical Centre to attend our clinics. If you are not already enrolled, come down to the paa and bring along any of the following documents and we can start that enrolment for you:

  • A form of photo ID such as:

    • Current New Zealand Passport

    • Current Drivers Licence

    • Current Community Service Card

    • 18+ card

  • Birth certificate

  • Proof of address - any letter with your name and address on it such as bank statement or power bill

If you are not sure your documents are ok, bring what you have anyway and let us help you sign up. The team can talk through what is needed when you arrive.
